Direct Care is a membership-based primary care and executive health program which provides comprehensive primary care services for a fixed monthly or annual fee.

The cost of membership is $395 per month or $4,500 per year when billed annually. This flat rate covers all of your care with Dr. Donlan and there are no surprise copays or deductibles.

Eligible HSA and FSA plans may be used towards the cost of your membership. We recommend consulting with your HSA/FSA provider and tax accountant for confirmation.

Dr. Donlan is out-of-network with all insurance plans. Dr. Donlan’s practice uses a membership model where patients are charged a fixed monthly fee which covers the cost of all primary care services rendered. See what’s included with membership details here.

If you have out-of-network benefits, Dr. Donlan can generate a super bill for you after each visit to submit to your insurer.

We still recommend carrying health insurance for medical care outside of our office including advanced imaging studies, medications, hospitalizations and specialist visits. Dr. Donlan is also able to bill your insurance for the costs of laboratory tests, imaging studies and immunizations not included in the membership fee.

Dr. Donlan will submit all laboratory testing to your insurance. We are partnered with LabCorp, Quest and Primex labs and work with their billing departments to ensure appropriate coverage.

Dr. Donlan will also utilize your insurance for advanced imaging studies. We are partnered with Mink Radiology and are able to offer expedited scheduling and results reporting to our patients. We are also able to send the orders to the imaging center of your choice.
